• Defendant's Name
• Case Number
• Date of Birth
• Type of Charges
• Include the type of document needed (i.e. affidavit of arrest (a-form), information/indictment, judgment and sentence, orders of probation-community control, motions, general orders, depositions)
• Mail appropriate fee: $1 per page.
• Indicate whether or not the copies should be certified;
• If certified there will be an additional cost of $2.00 per certification.
• Include requestor's name and address.
• Self-addressed stamped envelope.
• Scanned case files are available on CD disc for $42.00; (Felony Cases - Only). Additional research fees may apply.
